Final Step: Upload Images
To receive your free replacement product you must attach an an image of the recalled product with "RECALLED" written on the slats, an image of the law label, and proof of purchase. If for some reason you have trouble attaching these images, please email recall@lucidmattress.com for a solution.
By submitting this form, you agree to immediately stop using the recalled product and to destroy it. Orders are processed promptly in the order they are received. All orders sent ground shipping.
